Bariatric Endoscopy
What is a bariatric endoscopy?
Bariatric endoscopy refers to a group of minimally invasive procedures performed using an endoscope, a device consisting of a light and camera attached to a flexible tube, used to assist individuals with obesity in achieving weight loss. These procedures are designed to reduce the size of the stomach and limit food intake without the need for invasive surgery.
When is bariatric endoscopy indicated?
Bariatric endoscopy is a viable weight loss option for individuals whose BMI falls between 30 and 40 and who have not attained success through diet and exercise alone. This less invasive alternative to traditional weight loss surgeries can be performed as a day procedure, resulting in shorter recovery times and lower risks of complications. However, consulting with a healthcare provider to explore all weight loss options and determine the optimal approach based on individual needs and circumstances is crucial. Bariatric endoscopy may also be indicated if you have the following:
- A BMI (body mass index) of 27 or above with obesity-related comorbidities.
- Previous bariatric surgical procedures that didn't achieve the ideal weight loss goal.
- People who are overweight or obese have not been successful at losing weight with diet and exercise programs.
Types of Bariatric Endoscopy Weight Loss Procedures
For individuals who are ineligible for weight loss surgeries or prefer a minimally invasive and non-surgical approach, endoscopic weight loss procedures have been devised. Dr Andrew Viljoen is trained to perform the following weight loss bariatric endoscopy procedures:
- Intragastric balloon:
This is a procedure that involves the surgeon inserting a deflated balloon into the stomach through the mouth using an endoscope. After placement, the balloon is filled with a saline solution, leading to a decrease in the stomach's capacity to hold food.
An intragastric balloon is classified as either swallow or placement with an endoscope. Types of intragastric balloon placements:

- Allurion:
The Allurion Balloon is a weight-loss solution that can be performed by a healthcare professional who specialises in this area of medicine. The entire process takes less than 20 minutes. The patient is required to swallow a capsule that transforms into a gastric balloon. X-ray images are taken to determine whether the capsule is in the precise location. Dr Viljoen inflates the balloon using X-ray imaging for guidance. Once inflated, the balloon works by inducing a feeling of fullness, which helps curb hunger and promote healthier eating habits. Continuous follow-ups are part of the procedure to monitor weight loss. Read more

Orbera is an incision-less procedure that involves inserting the gastric balloon orally. Orbera allows patients to eat less and adapt to their new lifestyles by implementing healthier eating habits. The body’s response to the gastric balloon may be monitored within certain intervals three, four and six months after the treatment. Read more

Spatz3 is a unique gastric balloon system that is the only adjustable weight loss solution. With this particular weight loss treatment, the gastric balloon only takes 15 minutes to insert without creating any incisions. The problem with generic gastric balloons is that they lose their efficacy after four months, and patients develop balloon intolerance. This weight loss procedure is a minimally invasive procedure that utilises an endoscope to introduce sutures into the stomach, resulting in a tube-like structure that restricts the amount of food that can be ingested and slows down “gastric emptying”. - Endoscopic revision of bariatric surgery
(e.g. Gastric bypass): Endoscopic revision of bariatric surgery is a procedure used to revise or repair previous bariatric surgery. TORe is a revision of the gastric bypass procedure. TORe (Transoral Outlet Reduction) is a weight loss solution that reduces food intake by decreasing the size of the stomach outlet and is considered minimally invasive. It employs specialised tools and an endoscope to perform the procedure, resulting in fewer complications and a quicker recovery period compared to conventional surgical methods. is a non-surgical weight loss technique that utilises a lighted scope (endoscope) instead of creating external incisions over the body. Dr Viljoen inserts the endoscope orally through the mouth and guides it down the digestive tract. A suturing device is connected to the endoscope, which enables Dr Viljoen to restrict space within the gastric outlet, making it smaller. Read more
